Insurance

Insurance plays a role in any medical procedure or consultation: your hospitalization insurance, an occupational accident insurance, or a private insurance. Here's an overview of what you need to know and bring beforehand.
SECTION 1 — HOSPITALIZATION INSURANCE

Hospitalization insurance

Do you have hospitalization insurance? Depending on your policy, the invoice for your admission is sent directly to your insurance company, or you receive it at home yourself. Two scenarios:
SCENARIO 1

Receiving the invoice at home

You pay the invoice yourself first and submit it to your insurer afterward.
What to do?
  • Pay the invoice within 14 days
  • Request a reimbursement form
SCENARIO 2

Direct billing

AZORG bills the insurer directly. You only pay your personal contribution or deductible.
Mogelijk via:
  • Asurcard, Medi-link, DKV, ...

Notify your insurer in advance

Many hospitalization insurance policies require you to notify them no later than the day of admission. If you don't, your coverage may be reduced or even denied. At AZORG campuses, you can register your admission via the Assurcard kiosk in the entrance hall.

Note: Exclusions

What does your hospitalization insurance usually not cover?

  • Cosmetic procedures or treatments without medical necessity
  • Certain admissions for geriatric care, disability, or psychiatry (depending on your policy)
  • Outpatient invoices — consultations, examinations — are not sent directly to an insurer
TIP — SERIOUS OR SEVERE ILLNESS

Request a guarantee for serious illness

If you are diagnosed with a serious illness, your insurer can open a special file. Ask your urologist for a certificate.

SECTION 2 — OCCUPATIONAL ACCIDENT INSURANCE

Occupational accident

Is your consultation or admission the result of an accident at work or on your way to or from work? Always report this before your consultation or admission — this way you avoid the costs being billed to you personally.
Bring along

What information do we need?

Name and address of your occupational accident insurer

Policy number and file number

Name and address of your employer

Date of the accident

Provide this information to our secretariat (for consultations) or to the AZORG admissions department (for hospitalizations). Where possible, we will then contact the insurer or employer ourselves.

Note: What does an occupational accident insurer not cover?

Occupational accident insurers only pay according to the official RIZIV rates. They do not cover:

  • Fee supplements (since our doctors are not under agreement with the health insurance funds)
  • Supplements for a single room
  • Services outside the RIZIV nomenclature

These uncovered costs will be invoiced to you afterward.

SECTION 3 — SCHOOL OR SPORTS ACCIDENT

School or sports accident

Were you admitted or seen after an accident at school or during sports? Always report this before your consultation or admission — so we can prepare the correct paperwork.
Step 1

Via your health insurance fund

The medical costs are first processed through your health insurance fund — just like with a regular consultation.
Step 2

Personal contribution

You arrange the personal contribution yourself with the school's or sports club's insurance.

We are not able to handle the administrative steps — requests and authorizations for school or sports accident insurance billing.

SECTION 4 — PRIVATE INSURANCE (BELGIAN PATIENTS)

Private insurance

Do you have private insurance that covers outpatient care or hospitalizations? You can request a guarantee statement from your insurer in advance — this way you avoid having to pay all costs yourself upfront.

What do you need

Your identity card (eID)

A guarantee statement from your insurer

If your guarantee statement confirms that costs will be settled directly with the insurer, AZORG (for admissions) or our secretariat (for consultations) will send the invoice to the insurer. Without that statement, you pay the invoice yourself and can submit a reimbursement claim to your insurer afterward.

SECTION 5 — DUTCH PATIENTS

For patients with Dutch health insurance

Do you live in the Netherlands and choose care at Urology Group Aalst? Depending on your health insurer, billing is handled through different channels.
bring to your appointment

Which documents do you need?

Your ID

A guarantee statement from your insurer or your blue card (for care under the EU/S1 form or CZ card)

Direct billing to your health insurer

AZORG works directly with several Dutch health insurers (such as DVM and a.s.r.) — for hospitalizations, AZORG sends the invoice directly to them. Please arrange this with your insurer in advance.

Note: Not all care is fully reimbursed

Dutch health insurers apply their own policy with exclusions. Without a guarantee statement, you pay a combination or deposit, and you will receive the full invoice yourself afterward.
